Objectives

Goals are based on three guiding areas of skills development set by the Nursing Council. Goals: To understand the potential of the individual to respond to health conditions / disease; To Develop skills and knowledge to promote improvement in life quality in health / illness situations; To understand the coping of the patient / family dealing with chronic illness and / or disabling / suffering / death; To integrate teamwork in the context of providing continuous Care, based on the values and principles of an integrated approach ; To understand the specific requirements in Long Term Care and Palliative Care; To develop reflective analysis capabilities; To develop relational skills.

Program

Long Term Care: Conception, evolution, goals and types of services; Continuous Care Network in Portugal: Legal Framework; continuity of care, integrating care, individual process of continuous care, individual intervention plan, community outreach service, multidisciplinary; Reality of the Long Term Care / Azores; Functionality, Concepts and Dependency; Continuous Care and health promotion and of life quality for the client and informal casregivers; Continuous Care and home assistance: home care teams, Continuous Care and social support; The scope the of influence of the nurse of an Home Care Tema; Family Nurse , rehabilitation nursing in long term care;  Palliative Care National Program; needs of the patient at the end of life, symptom control, monitoring and communication with the patient end of life / family; grieving process; Stress and coping of the palliative care team.

Teaching Methodologies

Clinical Practice Project , Clinical Practice Report including a final reflection, case studies, data collection according to Nancy Rooper; with the pattern fill documentation: International Classification for Nursing (ICN)
